By creating an account with WA PCYC I agree to the following Terms and Conditions: In order to complete online registrations and enquiries you are required to register an account by providing us with accurate information including your name, address, a valid email address and telephone number. This will include linking any children to your parent account so as to perform transactions on their behalf. You warrant that any information that you provide while registering an account is accurate and correct at the time you provide the information, and that you will update your details when they change. You agree that you are legally entitled to provide this information to us and are the nominated parent/guardian for all minors that you also register. Your account will be protected by a password that is known only to you. Usernames and passwords are for your use only and must not be shared with any third party. We accept no liability for any activity that occurs in your account, including any purchases of Centre Services. PCYC is committed to the Privacy Act 1988 and the Health Records and Information Privacy Act 2002. For PCYC to conduct its activities and comply with government law and regulations, it is necessary to collect and use certain kinds of personal information about our participants. That information includes the information collected on this Enrolment form and because of being a participant. As much as possible, PCYC will only collect such information directly from you, but we may also collect information from a parent, guardian or third party in certain circumstances such as accidents or incident investigations.
